This is a fully remote position, open to applicants in California, +2 more states.
• Facilitate and coordinate high-quality, cost-effective care for patients with ESRD and CKD.
• Reduce fragmentation within the healthcare delivery system.
• Coordinate care among patients, families, physicians, providers, care teams, community resources, and the Regional Operations Manager.
• Provide support for ESKD and CKD patients across California, Oregon, and Washington.
• Evaluate patient needs and create individualized care plans while managing them effectively.
• Coordinate services across diverse interdisciplinary teams.
• Assist patients through complex transitions in healthcare.
• Oversee clinical, financial, and patient satisfaction outcomes.
• Utilize CQI techniques and engage in activities as a change agent.
• Adapt practice patterns to align with new electronic processes and Capella implementation.
• Travel within the assigned territory as needed.
• Achieve functional proficiency in DaVita clinical software programs, including Capella, within 90 days of employment.
• ADN degree from an accredited nursing school required; a three-year diploma from an accredited program may substitute for the nursing degree.
• BSN is preferred.
• Must possess a current Registered Nurse (RN) license in the state of practice; willingness to obtain licensure in California, Oregon, and Washington is necessary.
• Continuing education credits must be maintained as mandated by the state of practice.
• A minimum of five years of clinical nursing experience is strongly preferred.
• At least three years of renal nursing experience is preferred.
• Case management experience is required, or at least three years of experience in assessing patient needs, developing and managing individualized care plans, coordinating interdisciplinary services, and supporting complex healthcare transitions.
• Certification as a Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.
• Must have knowledge in managing clinical, financial, and patient satisfaction outcomes.
• Experience and effectiveness in a change-agent role are essential.
• Familiarity with CQI techniques is important.
• Experience in healthcare performance coaching is required.
• Current CPR certification is necessary.
• Ability to adapt to new electronic processes and meet increased productivity expectations for Capella implementation.
• Must hold a current driver’s license in the state of employment, have a positive driving record, and meet insurance coverage requirements.
• Basic computer skills and proficiency in Microsoft Word and Outlook are required.
• Must achieve functional proficiency with DaVita-specific clinical software programs, including Capella, within 90 days of employment.
• A home office with internet connectivity providing at least 1MB upload and 1MB download speed is necessary.
