Registered Nurse – Principal Care Management

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate patient records, medications, laboratory results, and care plans.

• Collaborate with healthcare providers, nursing personnel, pharmacies, and families as needed.

• Accurately and comprehensively document patient interactions.

• Recognize changes in patient conditions and escalate concerns appropriately.

• Support chronic disease management and provide patient education.

• Ensure that PCM services are performed in accordance with CMS regulations.

⛳️ Requirements

• A valid RN license is mandatory.

• Prior experience in nursing homes or long-term care settings is preferred.

• Familiarity with chronic care management or PCM is advantageous.

• Excellent communication and organizational abilities are essential.

• Must be comfortable working autonomously in a remote setting.

• Experience with EMR systems is preferred.
