This is a fully remote position, open to applicants in Illinois.
• Proactively assess patient vitals, symptoms, and pertinent clinical data to recognize care gaps and identify patients needing prompt intervention.
• Create, execute, and oversee personalized care plans that align with physician directives and individual health objectives.
• Communicate with patients through phone and secure messaging to resolve concerns, manage escalations, and provide compassionate, responsive clinical assistance.
• Conduct virtual follow-up appointments to support program onboarding, modify care plans, reinforce medication adherence, and assist patients in achieving lifestyle and health objectives.
• Accurately document all clinical interactions and prepare clear care summaries for patients’ primary care providers to ensure fluid coordination.
• Collaborate with interdisciplinary care teams to provide high-quality, patient-centered care for chronic conditions like CHF, hypertension, and type 2 diabetes.
• Contribute to the development and enhancement of clinical workflows and processes to foster efficient, scalable, and impactful care management programs.
• Embody Cadence’s dedication to exceptional care in all patient interactions, ensuring alignment with the standards and objectives of our health system partners.
• Multi-state RN Compact State Licensure.
• Active Illinois state RN license.
• Associate Degree in Nursing Science.
• A minimum of 5 years of recent clinical experience as an RN in an ICU/ER setting is preferred.
• Previous experience in supporting patients within a chronic care management program is a plus.
• To guarantee that our clinicians possess the essential tools for effective remote work, home office setups must have consistently stable wifi with robust upload and download speeds. A wifi speed test is required before the interview process to confirm that these standards are met.
• Proficient in nursing processes.
• Strong clinical acumen.
• Outstanding written, verbal, and interpersonal communication skills.
• Experience with EMR systems, preferably Athena and EPIC.
• Ability to work effectively with minimal supervision.
• Strong collaborative skills with cross-functional partners.
• Capability to support healthcare delivery to patients by performing a variety of activities and procedures as prescribed and directed by the Cadence Nurse Practitioner and Cadence clinical policies and procedures.
• Competent in patient assessment.
• Technically fluent with the ability to navigate multiple platforms and systems, including Notion, Athena, EPIC, Zendesk, and G Suite.
