This is a fully remote position, open to applicants in California, +2 more states.
• Oversee patient vital signs, symptoms, and laboratory data through the alert board to identify individuals requiring clinical intervention.
• Address incoming clinical inquiries and escalations from patients via phone and text messaging.
• Perform prompt outreach to patients with abnormal readings to evaluate clinical status and triage accordingly.
• Adhere to established triage protocols to determine subsequent actions, including provider escalation, after-hours care, emergency referrals, or activation of 911 when necessary.
• Accurately document all patient interactions, assessments, and interventions in the Cadence platform.
• Facilitate virtual follow-ups with patients to reinforce care strategies, medication compliance, and lifestyle objectives.
• Assist in care coordination and the creation of clinical summaries for patients' healthcare providers.
• Provide adaptable coverage across various clinical programs, including alert monitoring, CCM, and APCM, based on daily staffing requirements.
• Aid Cadence in scaling outstanding care delivery for patients with CHF, hypertension, diabetes, and other chronic health issues.
• Contribute to the ongoing enhancement of workflows and clinical processes.
• Play a key role in delivering an exceptional patient experience that aligns with Cadence’s care standards and health system objectives.
• Active RN compact multi-state license.
• Associate Degree in Nursing.
• Over 5 years of clinical nursing experience in outpatient, inpatient, chronic care, telehealth, or similar environments.
• Extensive experience in clinical triage, including telephone or virtual triage.
• Critical care or emergency nursing experience is highly preferred.
• Experience in supporting Chronic Care Management programs is preferred.
• Strong clinical judgment and the capacity to work independently.
• Ability to monitor patient vitals, symptoms, and lab results to identify care needs.
• Capability to follow up with patients to collect clinical information and triage effectively.
• Exceptional verbal, written, and interpersonal communication skills.
• Comfort in navigating multiple systems and technologies.
