This is a fully remote position, open to applicants in South Carolina, +1 more state.
β’ Initiate phone outreach to patients who have been discharged from designated units.
β’ Perform post-discharge follow-up calls within the timeframe mandated by CMS.
β’ Review discharge summaries, medication lists, and follow-up instructions prior to contacting patients.
β’ Offer medication reconciliation and coordination assistance.
β’ Evaluate patients' understanding of discharge instructions, symptom management, and adherence to medications.
β’ Document patient responses, advice given, variations, outreach attempts, interactions, and outcomes in the EHR.
β’ Identify potential complications and escalate any concerns to healthcare providers or the care team.
β’ Arrange primary care appointments and collaborate with front desk and clinical teams.
β’ Reinforce discharge education, including guidance on when to contact a provider or seek emergency assistance.
β’ Aid in preventing readmissions by ensuring follow-up and addressing care barriers.
β’ Review and address care gaps such as hypertension management, diabetes monitoring, preventive screenings, and wellness visits.
β’ Educate patients regarding overdue screenings or visits and assist them with scheduling.
β’ Collaborate with providers, care coordinators, and case management teams to ensure seamless continuity of care.
β’ Engage in team discussions and quality improvement initiatives.
β’ Ensure compliance with documentation and billing standards.
β’ Provide education focused on hypertension and intervene for elevated blood pressure readings or medication concerns.
β’ Collect patient data, complete necessary forms, identify needs and priorities, review diagnostic results, and perform advanced nursing observations.
β’ Develop follow-up care plans utilizing the nursing process in collaboration with patients, families, and other disciplines.
β’ Provide telephonic care advice and follow-up instructions in accordance with patient care standards.
β’ Perform additional duties as assigned.
β’ Associate degree in Nursing is required; a Bachelor's degree in Nursing is preferred.
β’ Minimum of one (1) year of experience as a registered nurse.
β’ Current RN compact/multistate license recognized by the NCSBN Compact State, or an RN license to practice in the state of employment.
β’ An RN diploma or certificate with the required current RN license may be accepted in lieu of an associate degree in nursing.
β’ Understanding and compliance with Prisma Health's mission.
β’ Capability to conduct post-discharge outreach calls, medication reconciliation, patient assessments, telephonic care advice, documentation, and follow-up coordination.
β’ Knowledge of age-specific patient needs, patient rights, confidentiality, ethical issues, and accepted nursing practice standards.
β’ Part-time employment.
