This is a fully remote position, open to applicants in South Carolina, +1 more state.
• Initiate phone outreach to discharged patients within the timeframe mandated by CMS.
• Conduct inquiries regarding patient experiences and recovery, providing follow-up support to patients and their families.
• Review discharge summaries, medication lists, and follow-up instructions thoroughly.
• Perform medication reconciliation and offer coordination support.
• Evaluate patients' understanding of discharge plans, symptom management, and adherence to prescribed medications.
• Document patient responses, advice given, outreach attempts, interactions, and outcomes in the Electronic Health Record (EHR).
• Identify potential complications and escalate any concerns to healthcare providers or the care team.
• Schedule primary care appointments and coordinate availability with front desk and clinical teams.
• Reinforce discharge education, including guidance on when to contact a provider or seek emergency care.
• Support efforts to prevent readmissions and address barriers to care.
• Review and manage open care gaps, including those related to hypertension, diabetes monitoring, preventive screenings, and wellness visits.
• Collaborate effectively with providers, care coordinators, and case management teams.
• Engage in team discussions and initiatives aimed at quality improvement.
• Ensure adherence to documentation and billing requirements.
• Provide education focused on hypertension and facilitate follow-up for patients with elevated blood pressure or medication-related concerns.
• Gather patient data, complete necessary forms, identify problems and priorities, and refer issues that require follow-up.
• Develop and assess follow-up care plans utilizing the nursing process, involving patients and their families.
• Perform additional duties as assigned.
• An Associate degree in Nursing is required.
• A Bachelor's degree in Nursing is preferred.
• A minimum of one (1) year of experience as a registered nurse is necessary.
• A current RN compact/multistate license recognized by the NCSBN Compact State, or a license to practice as an RN in the state of employment is required.
• An RN diploma or certificate may be accepted in place of an Associate degree in Nursing, provided the candidate holds the required current RN license.
• Knowledge of discharge care processes, medication reconciliation, patient follow-up, symptom management, and medication adherence is essential.
• Ability to identify complications, escalate concerns, and coordinate follow-up care effectively.
• Proficiency in documentation within the EHR in compliance with Prisma Health and CMS regulations is required.
• Strong critical thinking skills and advanced nursing observation abilities are necessary.
• Understanding of patient rights, confidentiality, ethical considerations, and age-related patient needs is important.
