This is a fully remote position, open to applicants in Virginia.
• Gathers delay and other relevant data for specific performance and outcome indicators. Aids in the collection and reporting of resource and financial metrics, including acute and post-acute case mix, length of stay (LOS), cost per case, excess days, resource utilization, readmission rates, and management of denials and appeals. Collects, analyzes, and addresses discrepancies from care plans and care pathways in collaboration with physicians and other healthcare team members. Utilizes concurrent variance data to drive practice adjustments and improve outcomes. Records significant clinical path variances and outcomes related to areas of direct responsibility, such as discharge planning and chronic disease management.
• Leverages pathway data in partnership with various disciplines to ensure effective patient management concurrently. Guarantees safe patient care by adhering to policies, procedures, and standards within budgetary constraints, including time management, supply oversight, productivity, and accuracy of practice. Encourages individual professional development by fulfilling requirements for mandatory and continuing education and skill competency. Supports department-focused goals that contribute to the organization's overall success.
• Facilitates discharge planning and continuity of care for assigned patients in both acute and post-acute environments. Initiates and coordinates referrals to clinics, home healthcare, hospice, skilled nursing facilities (SNF), acute rehabilitation, long-term acute care (LTAC), transitional care management (TCM), and medical equipment and supplies as necessary. Works collaboratively with the interdisciplinary healthcare team, patients, and families to assess and coordinate discharge planning needs, provide post-discharge services, and ensure a smooth transition from hospitals to discharge settings, as well as ongoing community care. Accurately documents pertinent discharge planning information in medical records in accordance with departmental standards and care management plans.
• Collaborates and communicates with both internal and external case managers. Possesses a thorough understanding of pre-acute and post-acute resources. Coordinates services and serves as a key liaison between patients, their families, and interdisciplinary healthcare team members. Works closely with healthcare team members to manage and coordinate all aspects of patient care. Adopts a holistic approach to ensure that healthcare and discharge plans address the physical, social, and emotional needs of patients.
• Certification: Basic Life Support (American Heart Association)
• Licensure: Licensed or eligible for licensure in the Commonwealth of Virginia as a Registered Nurse or possess an active multi-state Registered Nurse license
• Experience: A minimum of 1 year of experience in case management and/or clinical care
• Education: Bachelor’s Degree in Nursing or an Associate’s Degree. If the RN holds an Associate’s Degree, within 2 years of the hire date, they must meet with their nurse leader to: 1.) Identify the accredited school they intend to attend 2.) Provide a written plan outlining the anticipated BSN completion date 3.) Submit a review of transcripts from the institution indicating the required pre-requisites and the timeline for course completion 4.) Complete the BSN within 5 years of the start date.
