Registered Nurse Care Manager – Local Travel

This is a fully remote position, open to applicants in Maine.

📋 Description

• Act as a Registered Nurse within the Maine team, providing support to family caregivers.

• Perform initial in-home assessments prior to the commencement of caregiving services.

• Review and execute the Authorized Plan of Care as supplied by the state's Assessing Services Agency.

• Provide on-site oversight to caregivers at least once a month during the first three months of each family's participation in the program.

• Maintain precise clinical documentation and ensure compliance with EVV (Electronic Visit Verification) standards.

• Continue conducting initial and follow-up home visits for both new and existing patients.

• Train and assess caregiver competency while carrying out annual performance evaluations.

• Monitor patient health conditions and collaborate with service coordination agencies and physicians regarding any significant changes.

• Ensure consistent adherence to documentation standards and EVV requirements.

⛳️ Requirements

• Valid Registered Nurse (RN) license in the state of Maine.

• Preferred experience in home health, community health, or caregiver support.

• Excellent communication and interpersonal skills.

• Ability to work independently in a field-based setting.

• Reliable transportation for conducting home visits.
