
Registered Nurse – Care Manager

This is a fully remote position, open to applicants in Maine.
• Act as a Registered Nurse within our Maine team, providing support to family caregivers and ensuring the delivery of high-quality in-home care.
• Perform initial in-home assessments prior to the commencement of caregiving services.
• Review and execute the Authorized Plan of Care provided by the state’s Assessing Services Agency.
• Offer on-site supervision to caregivers at least once a month during the first three months of each family’s enrollment in the program, followed by supervisory visits every 3-6 months thereafter.
• Ensure precise clinical documentation and adherence to EVV (Electronic Visit Verification) standards.
• Continue initial and supervisory home visits for both new and existing patients.
• Educate and assess caregiver competence and perform annual performance evaluations.
• Monitor patients' health status and collaborate with service coordination agencies/physicians regarding significant changes.
• Uphold continuous compliance with documentation standards and EVV requirements.
• Valid Registered Nurse (RN) license in the state of Maine.
• Preferred experience in home health, community health, or caregiver support.
• Excellent communication and interpersonal abilities.
• Capability to work independently in a field-based setting.
• Reliable transportation for conducting home visits.
