This is a fully remote position, open to applicants in Arizona, +2 more states.
• Collaborate with a dedicated team to support a significant patient panel alongside Primary Care Providers (PCPs).
• Triage incoming communications, including phone calls, faxes, emails, and instant messages from teammates, patients, and partners.
• Function as a support system and escalation resource for LVN/LPNs within the team, improving our team's efficiency and effectiveness.
• Engage in thorough chart preparation and provide essential patient education regarding care plan modifications.
• Manage post-appointment activities, including diagnostic orders, referrals, and medication orders.
• Participate in daily huddles, offering guidance and promoting teamwork among your PCPs and Medical Assistant teammates.
• Carry out virtual patient assessments, creating personalized care plans and monitoring health outcomes.
• Act as a crucial communication link between patients, PCPs, specialists, and other healthcare professionals.
• Advocate for patient health by educating on preventive care and self-management strategies in alignment with the patient’s care plan and preferences.
• Engage in crisis intervention and palliative care discussions, prioritizing patient comfort and quality of life.
• Contribute to ongoing quality improvement initiatives and research efforts aimed at enhancing geriatric care.
• Possession of a current unrestricted multi-state RN license with a minimum of 2 years of clinical experience, ideally in geriatrics or related fields.
• Proven experience in remote roles, demonstrating proficiency with technology and digital communication platforms.
• A history of high accountability, outstanding communication skills, and the ability to excel in a fast-paced environment.
• Experience working with geriatric patients, particularly in environments such as senior living communities or skilled nursing facilities.
• Familiarity with chronic care management and a strong understanding of geriatric-related diseases and treatments.
