This is a fully remote position, open to applicants in California.
• Proactively oversee patient vitals, symptoms, and pertinent clinical data to detect care gaps and identify patients needing timely intervention.
• Create, implement, and manage personalized care plans that align with physician recommendations and individual health objectives.
• Communicate with patients through phone and secure messaging to address their concerns, manage escalations, and provide empathetic, responsive clinical support.
• Conduct virtual follow-up visits to facilitate program onboarding, modify care plans, reinforce medication adherence, and assist patients in achieving their lifestyle and health objectives.
• Accurately and efficiently document all clinical interactions, preparing clear care summaries for patients’ primary care providers to ensure seamless coordination.
• Collaborate with interdisciplinary care teams to provide high-quality, patient-centered care across chronic conditions such as CHF, hypertension, and type 2 diabetes.
• Participate in the development and enhancement of clinical workflows and processes to support efficient, scalable, and impactful care management programs.
• Uphold Cadence’s dedication to outstanding care in every patient interaction, aligning with the standards and objectives of our health system partners.
• Multi-state RN Compact State Licensure along with a single state RN license in California.
• Associate Degree in Nursing Science.
• A minimum of 5 years of recent clinical experience as an RN in an ICU/ER setting is preferred.
• Previous experience in supporting patients within a chronic care management program is advantageous.
• To ensure our clinicians have the essential tools for a successful remote work environment, home office setups must maintain consistently stable Wi-Fi with strong upload and download speeds. A Wi-Fi speed test is required prior to participating in the interview process to confirm that these standards are met.
• Proficient in nursing processes.
• Outstanding clinical acumen.
• Exceptional written, verbal, and interpersonal communication skills.
• Experience with EMR systems, preferably in Athena and EPIC.
• Ability to work effectively with minimal supervision.
• Strong collaboration skills with cross-functional partners.
• Capability to assist in delivering health care to patients by performing various activities and procedures as prescribed by and under the direction of the Cadence Nurse Practitioner and according to Cadence clinical policies and procedures.
• Competence in patient assessment.
• Technical fluency with the ability to navigate multiple platforms and systems, including Notion, Athena, EPIC, Zendesk, and G Suite.
