This is a fully remote position, open to applicants in United States.
• Provide exceptional, patient-oriented Medical Nutrition Therapy (MNT) for individuals with chronic and intricate conditions such as diabetes, cardiovascular disease, kidney disease, obesity, cancer, and gastrointestinal issues.
• Conduct thorough virtual assessments and create personalized nutrition care plans that align with patient needs, preferences, and objectives.
• Generate nutrition prescriptions customized to clinical diagnoses while encouraging sustainable behavior modifications.
• Record all patient interactions in the electronic health record (EHR) with clarity and timeliness.
• Work in collaboration with care teams, including physicians and health coaches, to ensure integrated patient care.
• Engage in the development of content for educational purposes, engagement strategies, and outreach initiatives.
• Participate in ongoing quality improvement efforts and innovation in care delivery.
• Utilize current nutrition research and evidence-based practices in patient treatment.
• Maintain necessary professional credentials, licenses, and pursue ongoing education.
• Join cross-functional meetings and contribute to strategic projects when suitable.
• Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration, in good standing AND a Master’s Degree in Nutrition or a related field preferred; OR a Bachelor's Degree with RD/RDN status obtained before the Master’s degree requirement takes effect (January 1, 2024).
