This is a fully remote position, open to applicants in New Hampshire.
• Provide exceptional, patient-focused Medical Nutrition Therapy (MNT) for individuals with chronic and complex health issues, including diabetes, cardiovascular diseases, kidney disorders, obesity, cancer, and gastrointestinal conditions.
• Conduct thorough virtual assessments and create personalized nutrition care plans tailored to each patient's needs, preferences, and objectives.
• Develop nutrition prescriptions that align with clinical diagnoses and encourage sustainable lifestyle changes.
• Accurately document all patient interactions in the electronic health record (EHR) in a timely and clear manner.
• Work collaboratively with care teams, including physicians and health coaches, to ensure coordinated patient care.
• Contribute to the development of educational content for outreach, engagement, and patient education.
• Engage in ongoing quality improvement initiatives and innovate care delivery methods.
• Utilize current nutrition research and evidence-based practices in the delivery of patient care.
• Uphold necessary professional credentials, licenses, and engage in continuous education.
• Participate in cross-functional meetings and assist with strategic projects as needed.
• Active Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration, maintaining good standing.
• A Master's Degree in Nutrition or a related discipline is preferred; alternatively, a Bachelor's Degree with RD/RDN status obtained before the Master's degree requirement takes effect (January 1, 2024).
• Must hold a valid license in the state of New Hampshire.
• Excellent communication abilities and a firm dedication to patient-centered care.
• Comfortable utilizing digital tools and platforms for delivering virtual care.
• Skilled at working effectively with a diverse range of patient populations and health conditions.
• Strong skills in problem-solving, critical thinking, and organization.
• A proactive and collaborative approach, with the ability to navigate ambiguity in a dynamic environment.
