This is a fully remote position, open to applicants in Maine.
• Provide exceptional, patient-focused Medical Nutrition Therapy (MNT) for individuals with chronic and intricate conditions such as diabetes, cardiovascular diseases, kidney issues, obesity, cancer, and gastrointestinal problems.
• Conduct thorough virtual assessments and craft personalized nutrition care plans tailored to each patient's needs, preferences, and objectives.
• Create nutrition prescriptions customized to clinical diagnoses, encouraging sustainable lifestyle changes.
• Record all patient interactions in the electronic health record (EHR) accurately and in a timely manner.
• Collaborate with interdisciplinary care teams, including physicians and health coaches, to facilitate coordinated patient care.
• Engage in the development of content for education, engagement, and outreach initiatives.
• Participate in ongoing quality improvement and innovation in the delivery of care.
• Utilize the latest nutrition research and evidence-based practices in patient management.
• Maintain all necessary professional credentials, licensing, and engage in ongoing education.
• Attend cross-functional meetings and contribute to strategic projects as appropriate.
• Must hold a current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential from the Commission on Dietetic Registration, in good standing, AND a Master’s Degree in Nutrition or a related field preferred; OR a Bachelor's Degree with RD/RDN status achieved before the Master's degree requirement takes effect (January 1, 2024).
• Must be licensed to practice in the state of Maine.
