This is a fully remote position, open to applicants in New York.
• Provide thorough virtual care, encompassing patient evaluations, management of conditions, medication assessments, and necessary escalation, all while adhering to telehealth regulations and licensing mandates.
• Work collaboratively with interdisciplinary teams and leverage telehealth processes to ensure coordinated, continuous care, maintaining precise documentation of patient interactions.
• Instruct patients on preventive healthcare, management of diseases, and promoting healthy lifestyle choices to enhance health outcomes and inspire active involvement in their wellness.
• Deliver empathetic, culturally aware care while upholding professionalism, effective communication, and sound clinical judgment in every interaction.
• Comply with clinical protocols, regulatory guidelines, and organizational policies while engaging in quality enhancement efforts.
• Guarantee accurate, prompt documentation, protect patient confidentiality, and uphold high standards of safety, ethics, and privacy.
• Possession of an active, unrestricted current NY license as a Provider in the state where you will be delivering patient care.
• License must not have any clinical or board practice limitations.
• For NY State applicants: Minimum of 3600 hours of practical experience, or 3 years of clinical experience (as an autonomous provider), in a primary care or longitudinal care environment.
• Board Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant.
• Achieve autonomous provider status as mandated by the state of licensure within six months of fulfilling eligibility criteria.
• Active Drug Enforcement Administration (DEA) license required.
• Current Basic Life Support Certification for healthcare professionals.
• Competence in utilizing electronic medical record (EMR) systems.
