This is a fully remote position, open to applicants in United States.

📋 Description

• Collaborate with older adults to evaluate their nutritional requirements and create tailored, evidence-based nutrition care plans.

• Provide medical nutrition therapy and coaching focused on lifestyle medicine, emphasizing a whole-food, primarily plant-based approach, customized to each patient's health status, cultural background, and preferences.

• Assist patients in managing chronic conditions where nutrition plays a crucial role (such as diabetes, cardiovascular and renal health, as well as weight and metabolic health).

• Ground your efforts in the 4 M's of Age-Friendly care — What Matters, Medication, Mentation, and Mobility — ensuring every plan prioritizes the patient's most significant needs.

• Work in conjunction with our wider care team, including lifestyle/longevity nurse practitioners, behavioral health specialists, and primary care coordinators, to address the comprehensive needs of the individual.

• Contribute to the development of the clinical model itself: protocols, intake processes, outcome measures, and the overall patient experience.

⛳️ Requirements

• A **Registered Dietitian (RD)** or **Registered Dietitian Nutritionist (RDN)**, accredited by the Commission on Dietetic Registration, in good standing *(this credential is essential for providing and billing for medical nutrition therapy under Medicare)*.

• Valid state licensure/certification where applicable for practice.

• Experienced and passionate about caring for older adults.

• A natural coach: warm, inspiring, and adept at facilitating lasting change in individuals.

• Comfortable with telehealth practices and enthusiastic about the dynamic nature of building something innovative.
