This is a fully remote position, open to applicants in United States.
• Collaborate with families during scheduling and intake to ensure they are well-prepared for their appointments and understand what to expect.
• Execute reminder calls, texts, and outreach using innovative engagement strategies aimed at improving appointment attendance and minimizing no-show rates.
• Inform families about Flourish's services, treatment model, and admissions process.
• Assist patients and caregivers in downloading, setting up, and navigating the Flourish app.
• Help families complete pre-admission forms, including but not limited to consents, releases of information, and pre-intake surveys.
• Request, gather, and monitor guardianship documentation and other necessary records needed prior to admission.
• Identify and proactively address obstacles that may affect attendance, engagement, or readiness for care.
• Utilize motivational interviewing and engagement techniques to support families who may feel hesitant, overwhelmed, or uncertain about treatment.
• Work closely with Clinical Admissions Specialists, Clinical Access Specialists, and operational team members to ensure seamless transitions and timely progress through the admissions process.
• Track outreach efforts, documentation, and readiness milestones in line with organizational standards.
• Monitor upcoming appointments and prioritize outreach activities to optimize clinician utilization and appointment attendance.
• Contribute innovative ideas, workflows, and process enhancements aimed at boosting show rates, patient engagement, and operational efficiency.
• Assist with administrative and operational projects as necessary.
• A Bachelor's degree is required; experience in behavioral health, healthcare administration, social services, care coordination, patient navigation, or a related field is preferred.
• 4-8 years of relevant professional experience in healthcare, behavioral health, customer success, patient engagement, admissions, or care coordination.
• Direct experience working with patients, families, or caregivers.
• Strong organizational skills and meticulous attention to detail.
• Excellent verbal and written communication abilities.
• Comfortable navigating evolving workflows in a growing organization.
• Proficient in using technology platforms, telehealth tools, and electronic documentation systems.
• Proven ability to engage individuals from diverse backgrounds with empathy and professionalism.
