This is a fully remote position, open to applicants in United States.
β’ Oversee a panel of clients focused on preventing or managing chronic cardiometabolic conditions.
β’ Engage with clients via video calls, phone conversations, and electronic messaging.
β’ Maintain expertise in Diabetes Self-Management Education and Medical Nutrition Therapy.
β’ Employ Motivational Interviewing Techniques to facilitate behavior change.
β’ Work collaboratively with other providers within the coordinated care team to deliver suitable care and an exceptional member experience.
β’ Given the sensitive nature of health information, uphold strict confidentiality at all times and meticulously adhere to HIPAA guidelines and processes to safeguard Protected Health Information (PHI).
β’ Comply with department policies and procedures.
β’ Stay current with Vida training requirements and communications.
β’ Achieve or surpass provider-level quality metrics and efficiency benchmarks.
β’ Bachelorβs or Advanced Degree from an accredited institution in dietetics or a related field.
β’ Registered Dietitian credentials through the Academy of Nutrition and Dietetics Commission on Dietetic Registration only.
β’ A minimum of 3 years of experience preferred in a clinical or community setting.
β’ Commitment of 10 hours per week.
β’ Certification as a Diabetes Care and Education Specialist is a plus.
β’ Certification in Obesity and Weight Management is a plus.
β’ Fluency in Spanish is a plus.
