This is a fully remote position, open to applicants in Florida.
• Delivers nutrition education and wellness coaching to enhance the member's ability to manage their health independently.
• Performs assessments, conducts surveys, and formulates an educational plan to provide health education to members.
• Develops a customized action plan that includes both short-term and long-term goals, prioritized by the member, along with specific activities and interventions to achieve these goals.
• Aids the member and their caregiver in understanding their condition, making lifestyle adjustments, adhering to prescribed medication, and collaborating with their healthcare provider to create a symptom response strategy. Engages with multidisciplinary teams.
• Recognizes and addresses obstacles that hinder the member from achieving their goals.
• Provides counseling on daily nutrition and medical nutrition therapy, reinforcing custom diet plans developed by physicians or nutritionists.
• Offers guidance on accessing community-based services for home modifications, transportation, meal preparation, etc.
• Emphasizes prevention, health promotion, maintenance, restoration, and education.
• Cultivates strong, trust-based relationships with individual participants.
• Supports clients in identifying, prioritizing, and implementing their choices. Empowers them to achieve their goals through guidance, encouragement, accountability, and counseling.
• Gives advice and instruction regarding fitness, nutrition, and the management of weight, stress, and overall health as part of participants' lifestyles.
• Commits to ongoing education in all areas pertinent to the role. Maintains and enhances knowledge about various health topics and behavior change techniques.
• Follows established workflows and collaborates with both internal and external stakeholders.
• Adheres to all policies and procedures to ensure that the program's outcome measurements comply with HEDIS and NCQA standards.
• Minimum of 5 years of relevant work experience with acute and chronic populations, providing nutritional and health education, self-management, wellness, and related support.
• A related Bachelor's degree in Food and Nutrition, Dietetics, or Health Education is preferred.
• Mandatory Registered Dietitian Certification upon hire.
• Experience in an outpatient clinical setting or primarily conducting telephonic nutrition health education and coaching is highly preferred.
• Experience in health behavior change, health coaching, or health education is advantageous.
• Exceptional interpersonal communication skills, with the ability to engage effectively with individuals from a diverse array of backgrounds and experiences in a professional, diplomatic, and courteous manner.
• Strong analytical and critical thinking abilities, including skills to identify issues, research and analyze problems from various perspectives, organize information, draw sound conclusions, and make appropriate decisions.
• Capable of handling protected health information while maintaining strict confidentiality and privacy in accordance with HIPAA regulations.
• Proficient in computer skills, including a solid understanding of Microsoft Office, Word, Excel, and the internet.
• Highly organized and efficient, able to work swiftly and with minimal supervision; a "team" player capable of thriving in a fast-paced, dynamic, and deadline-driven environment.
