This is a fully remote position, open to applicants in Ohio.
β’ Supervise facilities to guarantee assessments are carried out in accordance with established guidelines.
β’ Provide oversight during the facilities' weekly PDPM meetings for skilled patients.
β’ Monitor and track the effective use of PDPM tools, which include Physician Diagnosis Verification, Admission/Discharge CR(GG), Weekly CSR, PDPM Weekly Tracking Tool, and Point Right.
β’ Develop clinical reporting systems linking RDO and RDCS.
β’ Conduct monthly Regional MDS workshops focusing on PDPM and CMS-related regulations.
β’ Monitor and track QM and QASP performance, offering data analysis and clinical recommendations.
β’ Aid in the review of MDS/PDPM claims for accuracy and create clinical trend analyses.
β’ Deliver PDPM and MDS training to newly appointed MDS Coordinators.
β’ Establish performance evaluation systems for MDS Nurses in collaboration with facility Administrators.
β’ Ensure that triggered resident assessment protocol areas are incorporated into residents' care plans.
β’ Evaluate and ascertain health status and care level for new admissions.
β’ Guarantee the precise and prompt completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant change assessments.
β’ Communicate the level of care for new residents to all relevant disciplines.
β’ Coordinate interdisciplinary efforts to complete the Minimum Data Set (MDS) for new admissions within regulatory time limits.
β’ A Bachelor's Degree in Nursing (BSN) or an equivalent accredited nursing program degree.
β’ MDS certification is preferred.
β’ Experience in clinical and management positions, ideally in a regional or corporate environment.
β’ Familiarity with CMS/OIG surveys and regulations.
β’ Strong analytical and reporting capabilities.
β’ Excellent communication and interpersonal abilities.
β’ Capacity to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
β’ Proficiency in applying concepts such as fractions, percentages, ratios, and proportions to real-world situations.
β’ Ability to tackle practical problems and manage a variety of concrete variables in situations where standardization is limited.
β’ Skill in interpreting written, oral, diagrammatic, or scheduled instructions.
β’ Proficiency in Microsoft Suite products.
β’ Ability to occasionally lift and/or move up to 25 pounds.
β’ Extended use of a desktop or laptop computer.
