• Oversee the RCT Supervisors responsible for Care Management and the clinical tasks executed by the Regional Care Team.
• Continuously advocate for and educate on Mental Health/Substance Use (MH/SU) conditions and the most effective treatment practices.
• Ensure active participation in customized care management, working closely with members, healthcare providers, and their support systems to resolve issues, concerns, and obstacles to treatment and services/supports.
• Attend weekly meetings with Regional Managers, participate in Regional Leadership Team meetings, and engage in other departmental and organizational gatherings.
• Evaluate referrals to the Regional Care Team to confirm TCM eligibility and authorization, and assign them to the most suitable regional team.
• A Master’s degree in a discipline associated with Health, Psychology, Sociology, Social Work, Nursing, or another pertinent Human Services field, or licensure as a Registered Nurse (RN) along with three (3) years of experience in providing care management, case management, or care coordination for complex members with Intellectual/Developmental Disabilities (I/DD) or Traumatic Brain Injury (TBI); OR an equivalent combination of education and experience.
• Must be fully licensed by the North Carolina governing board that regulates Human Services professions, including: Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Addiction Specialist (LCAS), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Psychological Associate (LPA), or hold a Registered Nurse (RN) license from the North Carolina Board of Nursing.
• A valid driver’s license is required.
• Candidates must reside within the Trillium Catchment area, covering the designated counties.
• Ability to travel within the catchment area as needed.
• Preferred: At least two (2) years of experience in a supervisory capacity overseeing two (2) or more employees.
• Experience with children and adults who have Behavioral Health/Mental Health/Substance Use/Intellectual Developmental Disabilities (BH/MH/SU/IDD) conditions.
