This is a fully remote position, open to applicants in United States.
• Assess patient safety reports utilizing standardized classification systems.
• Facilitate investigations and action planning for incidents of harm, precursor events, and near misses.
• Conduct Apparent Cause Analyses (SWARM), contribute to and support Root Cause Analyses, and engage in Plan-Do-Study-Act (PDSA) cycles.
• Execute structured event interviews and reviews of medical records, ensuring psychological safety for participants during investigations.
• Deliver timely and clear summaries along with recommendations based on investigative findings.
• Lead or assist in Failure Mode and Effects Analyses (FMEA) to identify and mitigate risks before harm occurs.
• Perform gap assessments and design robust, sustainable system-level actions to address identified vulnerabilities.
• Employ reliable design principles, human factors, checklists, forcing functions, and other safety science practices to enhance care processes.
• Collaborate with clinical stakeholders to implement and monitor process modifications.
• Support the completion, rollout, and analysis of the Culture of Safety Survey.
• Facilitate data-driven action planning with leadership based on insights derived from the survey.
• Encourage harm reporting through a non-punitive framework and provide feedback that recognizes the value of reporting.
• Contribute to fostering a just, restorative culture by advocating for fair accountability and system-focused solutions.
• Analyze event data to detect patterns, trends, and emerging risks.
• Generate insights and recommendations for leaders within assigned Centers of Excellence or regions.
• Assist with or conduct basic data analysis to track safety performance and system outcomes.
• Communicate findings clearly and concisely to internal stakeholders.
• Educate clinicians, leaders, and staff on high reliability principles, human-centered resilience engineering, effective communication, sustaining situational awareness, and core patient safety concepts.
• Provide routine safety training and support organizational Quality & Safety initiatives.
• Act as a consultant for safety-related inquiries, workflows, and improvement projects.
• Collaborate across clinical, operational, and leadership teams to enhance safe and effective care delivery and workflows.
• Assist in the design, implementation, and evaluation of improvement projects aimed at reducing preventable harm.
• Participate in the Quality & Safety Committee and other relevant workgroups.
• Bachelor’s degree in a clinical or health sciences discipline (e.g., nursing, pharmacy, respiratory therapy, etc.)
• Over 3 years of clinical healthcare experience.
• At least 1 year of experience in healthcare quality, patient safety, and/or performance improvement.
• Proficient in data utilization, trending, and reporting.
• Experience in supporting or leading change initiatives.
• Familiarity with facilitating or participating in patient safety investigations (ACAs/SWARMs, RCAs, case reviews, etc.).
