PRN Pharmacist – Population Health

This is a fully remote position, open to applicants in Texas.

📋 Description

• Create, implement, and assess population health initiatives focused on medication management, adherence, and optimizing therapeutic outcomes.

• Leverage health data analytics to pinpoint trends, care gaps, and intervention opportunities to enhance medication-related results for the target population.

• Partner with healthcare providers to ensure suitable medication choices, dosing, and monitoring, taking into account population-specific needs and evidence-based guidelines.

• Develop and execute programs to educate patients about their medications, ensuring comprehension, adherence, and management of potential side effects.

• Collaborate closely with healthcare teams, including physicians, nurses, social workers, and other professionals, to synchronize care plans and interventions for improved patient outcomes.

• Engage in quality improvement initiatives aimed at enhancing medication-related outcomes, minimizing adverse events, and promoting evidence-based practices.

• Remain informed about pharmaceutical regulations, ensuring adherence to applicable laws, policies, and guidelines.

• Provide leadership within the pharmacy team, mentor junior staff, and contribute to departmental goals and initiatives.

⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution.

• Active pharmacist licensure in the state or region of practice, with an additional active pharmacist license in Texas.

• Advanced training or certification in population health management or a related field (preferred).

• Strong analytical skills and proficiency in data interpretation.

• Experience in pharmacy practice, ideally in a population health or managed care environment.

• Exceptional communication, collaboration, and leadership skills.

• Understanding of healthcare regulations, policies, and quality improvement methodologies.

• Proficiency in Spanish (preferred).
