This is a fully remote position, open to applicants in Illinois, +2 more states.
• Deliver Comprehensive Medication Management (CMM) in partnership with hub-based and virtual clinical care teams, and provide additional pharmacist support services as required.
• Engage in the formulation of a medication management action plan aimed at achieving member self-management objectives and clinical outcomes, complete with measurable metrics and a timeline for monitoring and follow-up.
• Collaborate and communicate effectively with Cityblock and external providers within the member care team to ensure that all clinical interventions and encounters are in alignment with medication-related objectives, while conducting ongoing evaluations and adjustments to the care plan necessary for goal accomplishment.
• Offer direct assistance to clinical team members regarding general drug information, dosing, avoidance of interactions/adverse effects, or any member-specific requirements.
• Take part in staff training by presenting medication management in-services, developing educational resources for both clinical and non-clinical team members, and training/coaching/facilitating all roles on the consistent upkeep of a member’s best possible medication list care management platform, Commons, as well as the post-discharge medication reconciliation process for providers.
• Assist in the creation of medication management and disease state-specific clinical pathways/protocols, ultimately aiming to transition to collaborative practice agreements (CPA) with Cityblock physicians, allowing pharmacists to conduct member assessments, order laboratory tests, and initiate or modify medications per protocol to enhance the efficiency and effectiveness of team-based care.
• A valid Pharmacist license in at least ONE of the following U.S. states (MI, IL, OH)
• Willingness and eligibility to obtain Ohio licensure upon hire (company-supported and reimbursed)
• Completion of a PGY1 pharmacy practice residency and a PGY-2 Ambulatory Care Pharmacy Residency, or a minimum of 2 years of experience as a clinical pharmacist in an ambulatory care environment
