This is a fully remote position, open to applicants in Texas.
• Assess physician medication orders for suitability, accurate dosage, possible drug interactions, and appropriate administration routes.
• Offer clinical recommendations to physicians and healthcare staff concerning adjustments and optimizations in medication therapy.
• Keep current patient/resident profiles updated, which includes medical history, laboratory results, and existing medications (prescription, OTC, and home remedies).
• Verify that laboratory tests necessary for monitoring high-risk medications are correctly ordered, assessed, and communicated to the healthcare team.
• Work in partnership with physicians, nurses, and other healthcare professionals to deliver clinical and technical support.
• Record all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.
• Develop and enforce policies and procedures for the safe handling, dispensing, and clinical oversight/management of medications.
• Ensure adherence to all state and federal regulatory requirements related to pharmacy practice and medication management.
• Notify the Pharmacy Manager of any medication-related errors and adverse drug events as necessary.
• Provide education and support to the clinical team and healthcare agency staff on safe and effective medication handling practices.
• Carry out other pharmacy-related tasks and responsibilities as assigned.
• Bachelor’s Degree in Pharmacy; Pharm D is preferred.
• A minimum of one year of experience in long-term care is desired.
• Current and valid pharmacist license in the state of practice.
• Understanding of drug therapy guidelines, medication safety protocols, and regulatory standards (CMS, DEA, FDA).
• Proficient in electronic medical records and pharmacy information systems.
• Strong clinical judgment along with effective written and verbal communication skills.
