This is a fully remote position, open to applicants in United States.
• Execute various elements of the nursing process while adhering to safe nursing practices.
• Assume a supervisory role as required by Ace Care and its immediate needs.
• Deliver case management services for medically vulnerable and compromised patients utilizing best practices in Case Management.
• Conduct comprehensive MDS assessments for both new and renewing members.
• Responsible for assisting in the evaluation of member assessments, the implementation, and management of nursing services within the ACE Care framework.
• Initiate and update disease management as necessary.
• Assist in the initiation, development, and maintenance of care plans.
• Identify the needs of families/clients and provide supportive care in alignment with the care plan.
• Educate to uphold standards of care within the nursing and case management departments.
• Maintain consistent communication with the Administrator, other RN and LPN staff, and provide assistance regarding client progress and issues.
• Be available via phone or in person to address medical emergencies.
• Follow up on compliance with physician’s orders and treatments administered by direct care providers.
• Communicate with the client’s physician, family members, and the client regarding any changes in the care plan or services.
• Assist with the review of the Q.I program as needed.
• Support and attend provider meetings as necessary.
• Participate in IDT weekly meetings and provide support.
• Represent the assessment department at the WAM (Weekly Ace Meeting).
• Take responsibility for coordinating the physical care of clients or providing direct care as required.
• Understand and adhere to the policies and procedures governing the EDWP, formerly known as the SOURCE program.
• Operate independently in the absence of the administrator to maintain short-term operations.
• Accept additional assignments as appropriate.
• RN Certification (LPN or RN)
• Minimum of two (2) years of experience in a related field
• Proficiency in Microsoft 365, with strong skills in Microsoft Outlook, Word, and Excel
