Primary Care Physician, FM/IM

This is a fully remote position, open to applicants in Oregon.

📋 Description

• Deliver exceptional preventative, acute, and chronic care in a fully-remote, virtual environment.

• Enhance remote healthcare by fostering compassionate and impactful patient interactions.

• Oversee a dedicated group of patients as their primary care physician.

• Refer patients to specialists or acute care services as clinically necessary.

• Engage in population health initiatives for patients attributed to risk (AWVs, care gaps, transitions of care, risk capture).

• Assist patients immediately after discharge to enhance their understanding of treatment plans and minimize hospital readmissions.

• Conduct continuous self-assessment to uphold adherence to clinical guidelines and best practices.

⛳️ Requirements

• Board-Certified in Family or Internal Medicine.

• Over 3 years of experience post-residency.

• At least 2 years of experience in an in-person primary care environment.

• Experience with virtual visits is mandatory.

• Must possess an active OR state medical license; IMLC preferred.

• Prior experience with EPIC EMR is required.

• Clean background and no history of medical malpractice.

• Willingness to commit to 40 hours per week, with 36 hours dedicated to clinical practice.

• Availability to work on Monday and Friday with flexible start times (e.g., 7 am-12:30 pm PDT).

• Spanish speaking is advantageous.

• Strong desire to build long-term relationships with patients and manage chronic care conditions.

• Comfort in handling acute conditions within a virtual context.

• Extensive experience managing 3+ cases per hour.

• Eagerness to learn and implement innovative technology to enhance efficiency and clinical outcomes.
