This is a fully remote position, open to applicants in California.
• Deliver exceptional virtual primary care via video consultations, encompassing assessment, diagnosis, treatment, and patient education.
• Oversee a patient panel, providing comprehensive care for acute, chronic, and preventive health needs.
• Collaborate with the Care and Case Management team to formulate care plans and ensure successful patient outcomes.
• Partner with supervising physicians as stipulated by state and practice regulations.
• Coordinate with the Credentialing Team to secure additional state licenses as necessary.
• Uphold patient satisfaction and quality metrics by employing evidence-based medicine and demonstrating an exemplary bedside manner.
• Leverage telehealth technology to provide patient care while effectively managing multiple responsibilities in a fast-paced clinical setting.
• Remain updated on medical knowledge, treatments, and medications.
• Comply with assigned schedules, which may include evenings and weekends.
• Strictly follow HIPAA and security protocols to protect patient information.
• Foster a culture of humility, curiosity, and collaboration.
• A minimum of four years of experience in providing primary care in an ambulatory or outpatient family medicine or internal medicine setting.
• Proven experience in independently managing a patient panel as the designated primary care provider.
• Active, unrestricted Nurse Practitioner and RN licenses in the state of residence, with the capability to obtain licenses in additional states as needed.
• Multi-state licensure is highly preferred.
• Current and active board certification from a national certifying body through ANCC or AANP.
• Must maintain active Medicare status and not be excluded, sanctioned, or reprimanded by any state or federal healthcare program (including Medicare/Medicaid), the OIG, or the GSA.
• Comfort in addressing a wide range of medical conditions, including both acute and chronic issues as well as preventive care.
• Willingness and ability to treat patients of all ages across the lifespan.
• A professional, private, and secure home office environment for virtual patient consultations is essential.
• Proven ability to provide empathetic and compassionate care and support to patients and families during difficult times.
• Capacity to manage high patient volumes and competing priorities in a dynamic clinical environment while maintaining quality of care.
• Strong proficiency with technology and electronic health platforms; experience with telehealth is preferred.
