Population Health Pharmacy Technician, Fluent in Vietnamese

This is a fully remote position, open to applicants in Texas.

📋 Description

• Assist with the medication reconciliation processes for patients within the specified demographic.

• Collaborate with pharmacists and healthcare providers to enhance medication regimens.

• Review patient profiles to pinpoint potential medication-related issues and inconsistencies.

• Collect and analyze medication-related data to uncover trends and opportunities for enhancement.

• Generate reports on medication adherence, therapeutic outcomes, and other pertinent metrics.

• Work closely with healthcare teams to ensure effective coordination of pharmaceutical services.

• Communicate with patients, caregivers, and healthcare professionals to resolve medication-related issues.

• Engage in population health programs aimed at fostering medication adherence and preventive care.

• Support initiatives focused on enhancing health outcomes and minimizing medication-related disparities.

• Ensure compliance with regulatory standards, pharmacy laws, and organizational policies.

• Contribute to quality improvement initiatives within the pharmacy environment.

⛳️ Requirements

• High school diploma or equivalent; completion of a pharmacy technician program is preferred.

• Certification or licensure as a pharmacy technician in good standing.

• Certification or licensure in Texas.

• Strong understanding of pharmacy operations, medication management, and healthcare systems.

• Excellent communication, interpersonal, and organizational skills.

• Proficiency in using pharmacy software and computer systems for data entry and analysis.

• Ability to work collaboratively in a multidisciplinary team setting.

• Fluency in Vietnamese is required.
