This is a fully remote position, open to applicants in Oregon.
• Deliver comprehensive support to both patients and healthcare providers through a proactive approach aimed at enhancing patient care quality.
• Analyze and interpret clinician notes and patient assessments to gather necessary clinical data for prior authorization approvals.
• Create and refine clinical protocols specific to diseases and medications to optimize patient adherence and encourage evidence-based, cost-effective care.
• Track ongoing patient adherence across various sites and disease categories.
• Review electronic medical records (EMR) to assess drug and disease interactions, medication choices, laboratory results/vital signs, allergies, and prescription orders/renewals.
• Suggest alternative therapeutic drug regimens in line with established guidelines when appropriate.
• Monitor and assess therapies for side effects and other medication-related concerns.
• Conduct medication therapy management (MTM) sessions.
• Implement clinical interventions such as thorough pharmacist consultations, motivational interviewing/coaching, drug regimen optimization, and medication reconciliation.
• Track productivity and compile weekly statistics for developmental growth.
• Supervise the daily activities of technicians and ensure precision in delivery coordination slips and prior authorization submissions.
• Identify and address insurance-related issues, including situations that may require financial assistance.
• Oversee the communication of support personnel in compliance with HIPAA regulations.
• Maintain up-to-date registration and ensure all remote pharmacist activities comply with regulations.
• Facilitate direct communication with other healthcare professionals regarding patient care.
• Required Licensure/Certification: Applicants must possess a Doctor of Pharmacy (PharmD) or an equivalent advanced degree and be a licensed Pharmacist in the state of Oregon.
• A minimum of 1 year of experience in a clinical role is strongly preferred, though not mandatory.
