Population Health Pharmacy Technician

This is a fully remote position, open to applicants in Texas.

📋 Description

• Assist in the medication reconciliation processes for patients within the specified population.

• Collaborate with pharmacists and healthcare professionals to enhance medication regimens.

• Evaluate patient profiles to detect potential medication-related issues and discrepancies.

• Gather and analyze medication-related data to uncover trends and areas for improvement.

• Create reports on medication adherence, therapeutic outcomes, and other significant metrics.

• Work closely with healthcare teams to ensure smooth coordination of pharmaceutical services.

• Communicate with patients, caregivers, and healthcare providers to resolve medication-related issues.

• Engage in population health programs designed to encourage medication adherence and preventive care.

• Support initiatives aimed at enhancing health outcomes and minimizing medication-related disparities.

• Ensure adherence to regulatory standards, pharmacy laws, and organizational policies.

• Contribute to quality improvement initiatives within the pharmacy environment.

• High school diploma or equivalent; completion of a pharmacy technician program is preferred.

• Certification or licensure as a pharmacy technician (requirements may differ by location).

• Strong knowledge of pharmacy operations, medication management, and healthcare systems.

• Excellent communication, interpersonal, and organizational abilities.

• Proficiency in utilizing pharmacy software and computer systems for data entry and analysis.

• Ability to work effectively in a multidisciplinary team setting.

• Fluency in Spanish is preferred.
