Population Health Pharmacist – Spanish Speaking

This is a fully remote position, open to applicants in Texas.

📋 Description

• Develop, execute, and assess population health initiatives focused on medication management, adherence, and the optimization of therapeutic outcomes.

• Leverage health data analytics to uncover trends, identify gaps in care, and discover intervention opportunities aimed at enhancing medication-related outcomes for the designated population.

• Collaborate with healthcare providers to guarantee appropriate medication selection, dosing, and monitoring while considering the specific needs of the population and adhering to evidence-based guidelines.

• Create and implement educational programs for patients regarding their medications to ensure comprehension, adherence, and the management of potential side effects.

• Coordinate closely with healthcare teams, including physicians, nurses, social workers, and other healthcare professionals, to develop care plans and interventions that lead to improved patient outcomes.

• Engage in quality improvement projects that aim to enhance medication-related outcomes, reduce adverse events, and advocate for evidence-based practices.

• Keep abreast of pharmaceutical regulations, ensuring adherence to applicable laws, policies, and guidelines.

• Provide leadership within the pharmacy team, mentor junior staff, and contribute to departmental objectives and initiatives.


⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution.

• Active pharmacist licensure in the respective state or region of practice.

• Advanced training or certification in population health management or a related discipline (preferred).

• Strong analytical capabilities and skills in data interpretation.

• Experience in pharmacy practice, ideally in a population health or managed care environment.

• Excellent communication, collaboration, and leadership skills.

• Familiarity with healthcare regulations, policies, and quality improvement methodologies.

• Proficiency in Spanish (preferred).
