Population Health Pharmacist

This is a fully remote position, open to applicants in Texas.

📋 Description

• Develop, implement, and assess population health initiatives focused on medication management, adherence, and enhancing therapeutic outcomes.

• Leverage health data analytics to pinpoint trends, identify care gaps, and discover intervention opportunities to enhance medication-related results for the targeted population.

• Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring, taking into account population-specific needs and evidence-based guidelines.

• Create and execute programs that educate patients about their medications, ensuring comprehension, adherence, and management of potential side effects.

• Work closely with healthcare teams, including physicians, nurses, social workers, and other healthcare professionals, to coordinate care plans and interventions aimed at improving patient outcomes.

• Engage in quality improvement projects focused on enhancing medication-related outcomes, minimizing adverse events, and advocating for evidence-based practices.

• Remain informed about pharmaceutical regulations to ensure adherence to relevant laws, policies, and guidelines.

• Provide leadership within the pharmacy team, offer mentorship to junior staff, and contribute to departmental goals and initiatives.


⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution.

• Active pharmacist licensure in the state or region of practice.

• Advanced training or certification in population health management or a related field (preferred).

• Strong analytical skills and proficiency in data interpretation.

• Experience in pharmacy practice, preferably within a population health or managed care environment.

• Exceptional communication, collaboration, and leadership skills.

• Familiarity with healthcare regulations, policies, and methodologies for quality improvement.
