Physician Assistant/Psych Nurse Practitioner

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct telehealth follow-up appointments for managing psychiatric medications in elderly patients.

• Monitor treatment effectiveness and modify medications while considering age-related risks, polypharmacy, and medical comorbidities.

• Perform cognitive evaluations and screenings using tools such as MoCA, MMSE, or equivalent assessments.

• Create initial care coordination and behavioral support plans.

• Collaborate closely with nursing staff to ensure ongoing monitoring and patient support.

• Communicate with the supervising psychiatrist as necessary.

• Maintain prompt and precise documentation in the electronic medical records (EMR).

⛳️ Requirements

• Must be licensed and certified as a Physician Assistant or PMHNP in Pennsylvania (mandatory).

• Experience in geriatric psychiatry, long-term care, or dementia care is highly preferred.

• Familiarity with cognitive assessment tools and behavioral management techniques.

• Comfortable providing care through telehealth platforms.

• Strong clinical judgment regarding polypharmacy, fall risk, and medication safety in older adults.

• Ability to collaborate effectively within a team-based care approach.
