This is a fully remote position, open to applicants in California.
• Assess patients’ functional capabilities, mobility, muscle strength, gait, and range of motion to create tailored treatment plans.
• Provide physical therapy services in alignment with the physician’s care plan and in accordance with California’s Physical Therapy Practice Act.
• Inform patients and their families or caregivers about therapeutic exercises, equipment usage, and recovery objectives.
• Collaborate with physicians and the healthcare team, communicate changes in patient conditions, and engage in case conferences.
• Supervise Physical Therapy Assistants and, when physical therapy is the only skilled service, oversee Home Health Aide care on a bi-weekly basis.
• Oversee the documentation process, including clinical progress notes and discharge planning.
• Recognize needs for additional home health services and facilitate referrals when necessary.
• Execute other responsibilities as needed to support patient care.
• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
• Valid California Physical Therapy license.
• At least 2 years of physical therapy experience; experience in home health is advantageous.
• Excellent communication skills with the ability to engage effectively with patients and their families.
• Current CPR certification from the American Heart Association.
• Hold a valid driver’s license and possess a reliable, insured vehicle.
