This is a fully remote position, open to applicants in Philippines.
• Perform thorough evaluations of patient care documentation in accordance with established standards, protocols, and regulatory guidelines.
• Accurately complete and submit Minimum Data Set (MDS) assessments within specified time frames.
• Create and revise personalized care plans for residents.
• Ensure that assessments and documentation adhere to Medicare/Medicaid, CMS, and state regulations.
• Collaborate with nurses, therapists, and other healthcare professionals to enhance the quality of patient care.
• Maintain accurate documentation to support reimbursement and compliance needs.
• Utilize knowledge of the Patient-Driven Payment Model (PDPM) to facilitate correct reimbursement classification.
• Review patient records and clinical documentation for thoroughness and precision.
• Detect missing, incomplete, or inaccurate information and take appropriate follow-up actions.
• Provide constructive feedback and assist in ongoing education and training based on findings from documentation reviews.
• Manage multiple deadlines independently while working remotely.
• Partner with clinical staff, managers, and other stakeholders to pinpoint areas needing improvement.
• Must possess a current and valid PHRN license.
• A minimum of 1 year of bedside or clinical experience is required.
• At least 2 years of experience with Minimum Data Set (MDS) is necessary.
• Experience supporting a U.S. Skilled Nursing Facility (SNF) is preferred.
• Strong understanding of prehospital patient care protocols, standards, and regulatory requirements.
