PHRN Clinical Coordinator, MDS

This is a fully remote position, open to applicants in Philippines.

📋 Description

• Perform thorough evaluations of patient care documentation in line with established standards, protocols, and regulatory guidelines.

• Accurately complete and submit Minimum Data Set (MDS) assessments within specified timelines.

• Create and revise personalized care plans for residents.

• Ensure that assessments and documentation comply with Medicare/Medicaid, CMS, and state regulations.

• Collaborate with nurses, therapists, and other healthcare professionals to enhance quality patient care.

• Maintain accurate documentation to support reimbursement processes and compliance.

• Utilize knowledge of PDPM (Patient-Driven Payment Model) to ensure proper reimbursement classification.

• Review patient records and clinical documentation for completeness and precision.

• Identify and follow up on missing, incomplete, or incorrect information.

• Provide constructive feedback and assist with ongoing education/training based on documentation review outcomes.

• Manage various deadlines autonomously in a remote work setting.

⛳️ Requirements

• Possess a current and valid PHRN license.

• Have a minimum of 1 year of bedside or clinical experience.

• Must have at least 1 year of experience with MDS (Minimum Data Set).

• Experience working in a U.S. Skilled Nursing Facility (SNF) is required.

• Strong understanding of prehospital patient care protocols, standards, and regulatory requirements.

• Exceptional attention to detail and analytical capabilities.
