• Demonstrate an understanding of confidentiality in relation to patient/client care, adhering to all federal and state regulations concerning the confidentiality of protected health information (PHI) and electronic protected health information (EPHI), while following HIPAA guidelines for identifiable protected health information.
• Conduct telephonic follow-ups with members to offer medication management support for chronic disease patients.
• Oversee the delivery of medications to patients.
• Facilitate connections between patients and the Pharmacist for all clinical requirements.
• Manage patient orders to the point of Pharmacist verification, ensuring that the order is complete and primed for final verification.
• Execute assigned responsibilities related to medication management services.
• Access, input, and retrieve information through the Pharmacy’s computer network to maintain precise records.
• Secure refill authorizations from prescribers.
• Process and prepare claims for third-party insurance.
• Maintain accurate patient/client profiles.
• Provide medication management services by following prescribed workflows.
• Adhere to established procedures, rules, and regulations.
• Complete clear and concise documentation within Care Management programs.
• Professionally answer phone calls and direct customer inquiries to the appropriate staff.
• Accept prescriptions for filling and ensure that the Pharmacist in Charge (PIC) has the necessary data to process the orders.
• Address customer concerns in a composed manner, referring issues to the pharmacist as necessary and notifying them of any complications.
• Promote teamwork, professional services, and effective communication.
• Recognize and respect the scope and limitations of job responsibilities.
• Perform additional related tasks as assigned.
• High school diploma or equivalent qualification.
• Pharmacy technician license or pharmacy technician trainee license issued by the Texas State Board.
• Excellent communication and customer service abilities.
• Willingness to work a flexible schedule, including evenings and weekends.
• Capability to read and transcribe pharmaceutical information accurately.
• Ability to sit for extended periods and stand intermittently.
• Flexibility to work in an office environment and from home as needed.
