• Conduct Medication Therapy Management (MTM) interventions.
• Perform medication reconciliation and identify opportunities for optimization.
• Deliver patient-focused education regarding medications and disease management.
• Support initiatives for chronic care, including diabetes, hypertension, COPD, and CHF.
• Employ behavioral strategies such as motivational interviewing to enhance adherence.
• Collaborate with healthcare providers on treatment planning and medication modifications.
• Identify and address medication-related risks and discrepancies.
• Ensure timely and precise documentation of all patient interactions.
• Facilitate effective care transitions and continuous care coordination.
• Doctor of Pharmacy (PharmD) degree.
• Current pharmacist license in at least one of the following states: CA, OR, WA, AZ, NM, NV, UT, ID, MT, WY, KY, OK, AR, LA, WV, NH, VT.
• Willingness to obtain additional licensure as needed.
• Board certification (e.g., BCPS, BCGP) is preferred.
• Experience in: MTM delivery, ambulatory or outpatient care, chronic disease management programs, and patient engagement and education.
• Familiarity with programs such as CCM, TCM, and RPM is advantageous.
