This is a fully remote position, open to applicants in Arizona, +16 more states.
β’ Conduct Medication Therapy Management (MTM) interventions.
β’ Perform medication reconciliation and identify opportunities for optimization.
β’ Deliver patient-focused education regarding medications and disease management.
β’ Support initiatives for chronic care, including diabetes, hypertension, COPD, and CHF.
β’ Employ behavioral strategies such as motivational interviewing to enhance adherence.
β’ Collaborate with healthcare providers on treatment planning and medication modifications.
β’ Identify and address medication-related risks and discrepancies.
β’ Ensure timely and precise documentation of all patient interactions.
β’ Facilitate effective care transitions and continuous care coordination.
β’ Doctor of Pharmacy (PharmD) degree.
β’ Current pharmacist license in at least one of the following states: CA, OR, WA, AZ, NM, NV, UT, ID, MT, WY, KY, OK, AR, LA, WV, NH, VT.
β’ Willingness to obtain additional licensure as needed.
β’ Board certification (e.g., BCPS, BCGP) is preferred.
β’ Experience in: MTM delivery, ambulatory or outpatient care, chronic disease management programs, and patient engagement and education.
β’ Familiarity with programs such as CCM, TCM, and RPM is advantageous.
