This is a fully remote position, open to applicants in California.
• Delivering comprehensive direct patient care to individuals diagnosed with leukemia, lymphoma, and various solid tumor cancers.
• Supporting the management of adult patients undergoing oral chemotherapy treatments.
• Conducting clinical reviews, educating patients, and managing care for those receiving oral oncolytics.
• Offering drug information, optimizing pharmaceutical therapy, ensuring adherence to regulatory standards, and undertaking additional projects as required.
• Pharm.D. or an equivalent qualification from a recognized pharmacy school.
• Completion of a PGY1 and/or PGY2 residency or comparable practical experience.
• Active pharmacist license in the State of California.
• BLS certification as detailed in the Pharmacist Responsibilities for Cardiopulmonary Resuscitation (C.P.R.) and Code Blue Procedure: Pharmacy Policy and Procedure.
• Extensive knowledge of medical terminology as it pertains to drug applications.
• Thorough understanding of common disease states and associated pathologies.
• Capability to comprehend patient medical management strategies.
• Skill in assessing and applying current pharmacological and biopharmaceutical principles for selecting and utilizing drug products in a clinical environment.
• Ability to extract relevant information from patients or other sources and incorporate it into the patient care plan.
• Possession of the verbal and written communication skills necessary to fulfill the responsibilities of a pharmacist.
• Proficiency in teaching and acting as a preceptor for training initiatives.
• Ability to prepare medications for immediate administration.
• Competence in providing recommendations and monitoring drug usage.
• Knowledge of laws, regulations, and standards pertaining to pharmacy practice.
• Capability to adapt to changes within the work environment.
• Skill in prioritizing workflow effectively.
• Ability to evaluate medication regimens to address the therapeutic needs of patients.
• Willingness to learn and adapt to the use of automated information systems in daily practice.
• Proven competency in pharmaceutical calculations, compounding, and pharmacokinetics techniques.
• Strong patient-centered communication skills.
• Proficient computer skills necessary to perform job functions.
