• Takes an active role in delivering patient care by conducting thorough medication reviews and/or implementing targeted interventions for specific diseases.
• Collects essential data to enhance patient safety by verifying that each medication is indicated, effective, safe, and that the patient adheres to the prescribed therapy.
• Enhances patient-specific drug therapy and addresses therapeutic issues by evaluating the appropriateness of medications, determining effectiveness, and recognizing potential interactions or adverse effects.
• Evaluates relevant patient-specific medical conditions, prioritizes these conditions based on urgency and severity, and identifies necessary preventive and health maintenance concerns.
• May create a tailored drug therapy plan for individual patient issues and effectively supports recommendations with persuasive justification.
• Enhances patient care through comprehensive counseling on medications and disease conditions, assessing adherence, analyzing medication costs, reviewing immunization status, and collaborating with patients, providers, and pharmacies.
• Offers consultation to other healthcare professionals upon request.
• For Specialty Pharmacy Only: Conducts patient counseling and monitoring as mandated by the patient management program.
• For Specialty Pharmacy Only: Engages in the quality management program to support ongoing process improvement initiatives and quality enhancement projects.
• Supervises pharmacy students and residents, as applicable.
• Operates in a state of constant alertness and safety.
• Performs additional duties as assigned.
• Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited pharmacy school.
• No prior experience required.
• Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) certification must be obtained within 90 days of employment.
• Additional resuscitation certifications may be necessary depending on the department.
• State of Work Location: Illinois Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR).
• State of Work Location: Missouri Pharmacist - Missouri Division of Professional Registration.
• State of Work Location: Oklahoma Pharmacist - Oklahoma State Board of Pharmacy.
