Pharmacist

This is a fully remote position, open to applicants in Hawaii.

📋 Description

• Deliver comprehensive Medication Therapy Management (MTM) services

• Perform medication reconciliation and evaluate the effectiveness of therapies

• Provide patient-centered education regarding medications and chronic illnesses

• Assist in managing chronic diseases (e.g., diabetes, hypertension, COPD, CHF)

• Apply motivational interviewing techniques to enhance adherence and patient engagement

• Collaborate with healthcare providers to optimize medication regimens and care plans

• Identify and resolve issues and risks related to medications

• Accurately document patient interactions in accordance with program standards

• Facilitate care transitions and promote continuity of care initiatives

⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree

• Valid pharmacist license in Hawaii (HI)

• Openness to obtaining additional state licensure if necessary

• Background in Medication Therapy Management (MTM)

• Experience in ambulatory care or outpatient environments

• Proficiency in chronic disease management

• Experience in patient education and engagement

• Familiarity with care programs such as CCM, TCM, and RPM is advantageous
