This is a fully remote position, open to applicants in Florida.
• Manage after-hours phone calls and effectively address the needs of callers by providing outstanding customer service.
• Offer telephone guidance and education tailored to patient requirements.
• Respond to telephone inquiries regarding the services offered by Chapters Health System.
• Promote and clarify the services provided by Chapters Health System.
• Collect essential information and documentation to ensure patient records are complete, current, and accurate.
• Input and monitor patient referral data in the Electronic Medical Record (EMR) system.
• Provide reassurance to patients and their families over the phone.
• Assist in resolving inquiries and determine when an in-person visit is necessary.
• Organize in-person visits as needed or upon request.
• Support patients and families in reaching their desired levels of comfort.
• Utilize support, expert resources, or personnel to address complex situations.
• Record patient and family interactions in the EMR and communicate with the Interdisciplinary Group.
• Report technical issues to leadership and ensure resolution within the required timeframe.
• Be available to report to a designated company location during emergencies as required.
• Carry out other assigned responsibilities.
• A current Florida or multi-state license to practice as a registered nurse.
• Capability to apply for or obtain a multi-state RN license within 30 days of hire if not already possessed.
• Ability to apply for or obtain an RN license in Washington, D.C. within 30 days of hire.
• A minimum of one year of nursing experience is required.
• Hospice or hospital experience is preferred.
• Prior experience with an EMR/EHR system is necessary.
• Proficient verbal and written communication skills in English.
• Bilingual in Spanish is preferred.
• Reliable transportation to meet the needs of patients.
• Strong critical decision-making and problem-solving skills.
• Exceptional clinical and assessment abilities.
• Proficiency in using equipment with visual and auditory mechanisms.
• Excellent time management and prioritization capabilities.
• Ability to maintain professionalism and composure in stressful situations.
• Strong skills in patient service, information-seeking, listening, and knowledge transfer.
• Capacity to thrive in a fast-paced, collaborative team environment.
• Ability to leverage interpersonal skills for maximum caller reassurance and patient visits.
• A sense of urgency in daily responsibilities.
• Computer proficiency, including MS Office, online research, data entry, and navigation.
• Availability for shifts that accommodate extended operating hours.
• Capability to perform sedentary work with frequent interruptions.
• Ability to manage emotional stress while caring for terminally ill patients and their families.
• Capacity to perform essential physical tasks, including lifting patients or equipment, bending, pushing/pulling, and kneeling, with or without reasonable accommodation.
• Successful completion of position competency requirements.
• Consent to drug and/or alcohol testing following a conditional offer and ongoing adherence to the Drug-Free Workplace Policy.
• Verification of eligibility through the Florida Care Provider Background Screening Clearinghouse following a conditional offer and continued eligibility.
• Pay rate of $45.00.
