Per Diem Field RN – Home Health, IHSS, ILST

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Conduct in-home evaluations, oversee clinical monitoring, train caregivers, and provide skills-based instruction tailored to the client and program assignment.

• Engage across three distinct programs — skilled home health, In-Home Support Services (IHSS), and Independent Living Skills Training (ILST).

• Enjoy clinical autonomy in the field without fixed shift obligations, mandatory overtime, or on-call duties.

⛳️ Requirements

• Current, unrestricted Colorado RN license (or compact multistate privilege).

• At least 1 year of clinical nursing experience.

• Valid BLS/CPR certification.

• Reliable transportation, a valid Colorado driver's license, and current auto insurance.

• Willingness to travel within the Greeley and/or Northglenn service areas.

• Capability to pass background checks and meet required health clearances.

• Experience in home health, hospice, case management, or community health (highly preferred).

• Familiarity with brain injury, neuro rehabilitation, developmental disabilities, or geriatric populations (highly preferred).

• Proficiency with electronic documentation (highly preferred).

• Bilingual in English and Spanish (highly preferred).
