Peer Support Specialist, Bilingual Preferred

📋 Description

• Assist individuals in establishing achievable and measurable goals to track their success and encourage behavioral change.

• Deliver on-platform behavioral health services to children and adolescents utilizing the service.

• Conduct structured Wellness Coaching that includes prevention, wellness promotion, and psychoeducation through organized coaching sessions.

• Engage in care coordination and case management.

• Provide Behavioral Health/Welfare Coaching interventions.

• Create personalized care plans to effectively set goals and develop action plans for positive behavioral changes.

• Perform risk assessments and implement mitigation strategies.

• Enhance positive mental health by addressing factors that contribute to it.

• Reduce mental health issues by tackling the determinants of such problems.

• Collaborate with users to identify their challenge areas, recognize obstacles, and formulate a plan to reach their wellness objectives.

• Conduct screening and assessments, including mental health and wellness evaluations, assessments, and progress tracking.

• Identify individual users who are experiencing or are at risk of social, emotional, and/or behavioral challenges.

• Moderate content submitted by service users, adhering to established policies and procedures.

• Facilitate warm handoffs by communicating with users and healthcare providers as necessary.

• Participate in and contribute effectively to clinical supervision, training, and meetings.

• Keep accurate records according to Kooth’s policies and procedures.

⛳️ Requirements

• National Certified Peer Specialist.

• Certified Medi-Cal Peer Support Specialist.

• Six months of experience in, or volunteer experience providing, peer-to-peer support services.

• Current state certification with at least 40 hours of training.

• Must permanently reside in the contiguous 48 states of the US.

• Bilingual in Spanish is an advantage.
