• Engage with members who qualify for the TCL special needs population to offer education on permanent supportive housing, available supports, services, and other community resources.
• Collaborate with Diversion staff, Transition Coordinators, and the Next Step Rehousing Team to assist in transition planning and facilitate movement through the TCL process for members seeking permanent supportive housing within the community.
• Work alongside the Trillium Integrated Care Nurse, OT, COTA, and Clinician to assess complex care needs and determine clinical service linkages.
• Promote community integration, connect members to services, and provide education and support for those who opt not to transition to permanent supportive housing.
• Educate and support members and their guardians in understanding and exercising their rights and responsibilities.
• A High School Diploma or GED is mandatory.
• Must be an individual with at least one year of lived experience with a behavioral health condition and/or substance use.
• North Carolina Peer Support Specialist Certification is required.
• A valid driver’s license is essential.
• Candidates must reside within Trillium’s Mid State Region, which encompasses the following counties: Anson, Guilford, Montgomery, Randolph, and Richmond.
• Ability to travel within the designated catchment area as necessary is required.
