This is a fully remote position, open to applicants in Florida, +3 more states.
β’ As an Advanced Practice Provider (APP) with Imagine Pediatrics, you play a crucial role in ensuring top-notch clinical care for your patients.
β’ Oversee a panel of complex pediatric patients β Manage care plans, collaborate with primary care providers and specialists, and guarantee comprehensive care coordination.
β’ Collaborate within a multidisciplinary team β Partner with pediatricians, nurses, pharmacists, dietitians, and social workers to deliver integrated care in a virtual-first model.
β’ Empower families β Educate and support parents in navigating their child's health journey with assurance, fostering long-term health success.
β’ Provide high-quality virtual care β Conduct virtual enrollment visits, manage acute escalations, handle sick visits, perform post-hospital follow-ups, and conduct in-home virtual assessments.
β’ Ensure continuity of care β Intervene as necessary to bridge care gaps, coordinate prescriptions, and streamline referrals to eliminate barriers to care.
β’ A minimum of 5 years of experience working with a pediatric population is required, with a strong preference for Community-Based Health or Complex Care.
β’ Completion of an accredited Nurse Practitioner (NP) or Physician Assistant (PA-C) program, along with holding an unrestricted NP or PA state license and board certification.
β’ Active licensure in NJ, NC, FL, or NY is required; an additional license in GA or DC is preferred.
β’ Experience and comfort in working with children who have complex medical needs.
β’ Experience with Medicaid and/or community health is preferred.
β’ Virtual care experience is a plus.
β’ A strong desire to enhance the quality of life for pediatric patients and their caregivers.
β’ Capability to lead and guide a clinical team effectively.
β’ Bilingual in Spanish is strongly preferred.
