β’ Support the management of your patient panel's health and actively monitor their adherence to preventative care measures.
β’ Facilitate referrals, manage prior authorizations, and oversee lab order fulfillment.
β’ Conduct proactive follow-ups with patients regarding their vital signs (BP, BS, weight), symptom evaluations, and scheduling requirements.
β’ Assist patients with prescription fulfillment, initiation, and follow-up processes.
β’ Educate patients and their families about chronic conditions and strategies to prevent further disease progression.
β’ Ensure collaborative care delivery and engage specialists as necessary.
β’ Provide assistance to clinicians as needed.
β’ Perform additional duties as assigned.
β’ Strong time management skills with a proven ability to maintain high attention to detail.
β’ Experience with electronic health records (EHR); familiarity with Athena and Salesforce is preferred.
β’ Medical Assistant certification is a plus.
β’ Excellent communication skills with outstanding listening capabilities.
β’ Flexibility and availability throughout the workday as required.
