This is a fully remote position, open to applicants in Arizona, +1 more state.
• Support the management of your patient panel's health and actively monitor their adherence to preventative care measures.
• Facilitate referrals, manage prior authorizations, and oversee lab order fulfillment.
• Conduct proactive follow-ups with patients regarding their vital signs (BP, BS, weight), symptom evaluations, and scheduling requirements.
• Assist patients with prescription fulfillment, initiation, and follow-up processes.
• Educate patients and their families about chronic conditions and strategies to prevent further disease progression.
• Ensure collaborative care delivery and engage specialists as necessary.
• Provide assistance to clinicians as needed.
• Perform additional duties as assigned.
• Strong time management skills with a proven ability to maintain high attention to detail.
• Experience with electronic health records (EHR); familiarity with Athena and Salesforce is preferred.
• Medical Assistant certification is a plus.
• Excellent communication skills with outstanding listening capabilities.
• Flexibility and availability throughout the workday as required.
