Patient Enrollment Specialist – Contractor, Evening Shift

📋 Description

• Provide education to prospective patients and their families regarding the program.

• Support patients in using the program's app and/or website.

• Conduct outbound calls to reach out to potential patients.

• Evaluate patient eligibility and respond to questions and concerns.

• Accurately document and maintain information within the system.

• Meet weekly productivity targets.

• Ensure prompt follow-up for patients expressing interest.

• Keep updated on advancements in cardiac rehabilitation.

• Distribute educational materials to patients who show interest.
