Patient Care Coordinator

This is a fully remote position, open to applicants in Arizona, +2 more states.

📋 Description

• Act as the main contact for patients, their families, facility partners, and providers within your designated patient panel.

• Assess and prioritize urgent care requests, STAT tasks, and time-sensitive clinical communications in real time.

• Ensure accurate and compliant patient documentation is maintained within our EMR system.

• Proactively oversee appointment scheduling, follow-up care, and interdisciplinary communication.

• Lead the onboarding process for new patients, ensuring each one is contacted and scheduled within 24 hours of enrollment.

• Participate in after-hours on-call rotation to maintain continuity of care.

• Contribute to initiatives aimed at quality improvement and enhancing operational efficiency.

⛳️ Requirements

• A minimum of 2 years of direct patient care experience in a clinical environment (such as Medical Assistant, LVN/LPN, CNA, Phlebotomist, Patient Care Technician, or similar roles).

• A Bachelor's degree is strongly preferred.

• Experience in healthcare coordination, patient services, or clinical operations is essential.

• Strong skills in triage and prioritization within high-volume, real-time settings.

• Proficiency with EMR/EHR systems and healthcare technological platforms.

• Excellent communication skills when interacting with diverse stakeholders, including patients, families, physicians, and facility leadership.

• Self-motivated and accountable; capable of working independently in a fully remote environment.

• Must have reliable high-speed internet and a dedicated, private, HIPAA-compliant workspace at home.

• Must remain in good standing with OIG, CMS, and Federal Health Care Programs — no exclusions or debarments allowed.
